How Do We Use Your Gathered Inormation

  • Medical information to properly assess your health condition, diagnose and provide appropriate treatment accordingly.

  • Continuity of your health care by sharing it with your regular GP and other third party, such as: pathology and x-ray when required.
  • Feedback for the improvement of our provided after-hours health service, approved by you.
  • Comply with any legislative or regulatory requirements, i.e. notifiable communicable diseases.
  • Communicate with you on regular basis about our services using SMS, EMAIL or other means as approved by you.
  • If required by law, for example government bodies and law enforcement agencies.
  • You can notify us at any time if you no longer wish to be contacted by us.
Personal Information Shared With Doctors And Chaperones

Upon your booking, the personal information you provided will be shared with the relevant doctor in the area, and his chaperone.

According to our after-hours recruitment Policy and Procedure, it is a requirement that our doctors and chaperones sign our Privacy and Confidentiality Agreement; this is in accordance with the National Privacy Principles contained in the Privacy Act 1988. (Complying with APP Amendment of 12 March 2014), the ACT will be clearly explained prior to receiving their signature.

Call Home Visiting Doctor is not responsible for the practice of these individuals or organisations should any of them breach their commitment and legal obligation towards with the National Privacy Principles contained in the Privacy Act 1988. Use Of Your Information Outside Australia

Currently, our Call Centre is located on the Sunshine Coast and it is unlikely to use an overseas service provider for answering calls.

Patient’s Consent

By providing us your personal and medical information, you consent to:

 the collection, use and disclosure of this information for the purpose related to the provision of medical services by our afterhour’s doctors.

consultation reports written by our doctors for your home visit or clinic consultation, to be shared with your nominated regular doctor, for the continuity of your medical care.

You also consent to our transferring your gathered personal and medical information to countries or jurisdictions outside Queensland and/or Australia as required by legal bodies. If we take such action, i.e. transferring your gathered personal and medical information outside Australia, those countries might not have the same Privacy Policies as in Australia. However, we will require from the recipient, if adequate, to put controls in place to protect your information.

Making a Complaint

Mistakes happen and human error occurs. Indeed we want to know if you are concerned about any aspect of our delivered service. We believe that problems are best dealt with internally through the management of this after-hours health service. We will endeavour to respond to your queries, and resolve any complaints, as quickly as possible. We will investigate the matter and will provide you with a written response to your complaint within 30 days of receiving your complaint.
